Quiz form

A form with correct answers, points and scored outcomes.

A quiz form attaches correct answers and point values to questions, then grades each submission automatically and can show a different outcome message per score band. It is the same response model as a survey, with a scoring layer on top.

Common uses go well beyond tests: lead scoring (route hot leads to sales), product recommenders ("which plan fits you"), and learning checks. Outcome branching means one quiz can deliver many tailored results without separate forms.

Designing a fair quiz means unambiguous correct answers, balanced point weighting, and deciding whether to reveal answers immediately or at the end. Scoring should run server-side so the answer key is never exposed to the browser.
